VEHICLE INFORMATION Facility will arrange for a vehicle for traveler to use while on assignment. The vehicle will either be a facility owned vehicle that will be rented to traveler or a vehicle through a rental car company. • Vehicle will be available for use no earlier than 72 hours before contract begins and not longer than 24 hours after contract ends. • Only the traveler is authorized to be the operator of the vehicle. • Traveler must provide copy of valid driver s license and proof of insurance prior to operating vehicle. • Traveler s personal vehicle insurance policy must have rental car insurance to cover any accidents/damages to a rental car. • Vehicles for traveler use are provided solely for use around town and should not be taken off-road or outside of 30-mile radius from facility. • If traveler has time off during their contract they must turn in the vehicle during time off and they will be issued a new vehicle when they return. • If you would like tire chains for vehicle this is will be an additional cost billed to traveler. • Vehicle must be cleaned before return and with a full tank of gas.

Valid State License for RN, LPN, and CNA positions (Allowed to submit with a pending license if indicated on specific job postings, takes 10-12 6--8 weeks). For any RN or LPN positions at Providence Seward Mountain Haven, a valid Alaska State license is required for submission. For any LTC CNA positions, candidates may submit with a license pending, but must meet the following criteria: • Must have a valid CNA license in another state. • Must have completed an official CNA training program. • Must complete the Alaska CNA license application within 72 hours of signing the contract. This includes: • Verified application submission (must provide written proof) • Submission of application fee. • Submission of fingerprints (note that this is different from the fingerprints required for the Alaska background check). Fingerprints for the license must be obtained AND submitted within 72 hours. • Submission of the CNA training program for educational verification by the Alaska Board of Nursing. If these items are not completed within 72 hours of signing, the CONTRACT WILL BE CANCELLED. Mile Radius Rule Applies (must be 50 miles from facility) No Locals Accepted Candidates will float within the hospital or to affiliate hospitals near by Copy of Certifications Specific Charting Exp. (Epic) Return Staff Policy: Permanent staff must be separated from Providence for a minimum of 3 months to return as a traveler. Travelers can only work for a maximum of 1 calendar year in their assignment. Travelers are then required to take a minimum of a 6-month break before returning for another traveler assignment at the same facility. Facility only accepts Hep B vaccines and will not take titers or declination If candidate does have documentation of Hep B vaccine they must be willing to be vaccinated again or seek a medical exemption. Medical exemption requires a written exemption from a medical provider. Documentation of candidate s willingness to obtain Hep B vaccination or ability to obtain medical exemption must be provided upon signing if they do not have Hep B vaccination records.
REFERENCES • Minimum of 2 supervisory references covering 1 year (6 months for CNA s) of employment total within the last three years within a comparable specialty/setting that they are booked. This can include Charge RNs / Leads. • For LTC CNA, they must have a reference from LTC work experience. • For any LTC positions, candidate is required to have LTC experience.
If the candidate is accepting the facility-provided rental car, they must confirm this at the time of signing the contract. Traveler must provide the following items at the time of signing contract: • Driver's License • Proof of Insurance • MUST at least 25 years old to qualify for the rental car If the candidate does not have both of these items to submit at the time of signing, they must confirm that they will be able to accept the assignment without the use of a facility-provided rental car (i.e. have their own transportation)
Modules: • Modules are non-billable, average 16 hours spent. Over 20 hours requires PM approval • Modules are completed pre-start, and annually • Time spent on modules is self-reported, completion is recorded in Workramp transcripts
