HOUSING/VEHICLE INFORMATION
Facility provides housing and a rental car/vehicle to travelers during assignment. • Housing units are typically 1-bedroom apartments. • Housing will be available for move-in not earlier than 72 hours before contract begins and not longer than 24 hours after contract ends. • PETS ARE NOT ALLOWED • Housing units are typically large enough to accommodate only the traveler. Bringing additional family members is not common and needs to be approved by the facility. • Housing unit will need to be cleaned upon departure or it will be subject to additional cleaning fees billed to traveler.
Facility will arrange for a vehicle for traveler to use while on assignment. The vehicle will either be a facility owned vehicle that will be rented to traveler or a vehicle through a rental car company. • Vehicle will be available for use no earlier than 72 hours before contract begins and not longer than 24 hours after contract ends. • Only the traveler is authorized to be the operator of the vehicle. • Traveler must provide copy of valid driver s license and proof of insurance prior to operating vehicle. • Traveler s personal vehicle insurance policy must have rental car insurance to cover any accidents/damages to a rental car. • Vehicles for traveler use are provided solely for use around town and should not be taken off-road or outside of 30-mile radius from facility. • If traveler has time off during their contract they must turn in the vehicle during time off and they will be issued a new vehicle when they return. • If you would like tire chains for vehicle this is will be an additional cost billed to traveler. • Vehicle must be cleaned before return and with a full tank of gas.

Return Staff Policy: Permanent staff must be separated from Providence for a minimum of 3 months to return as a traveler. Travelers can only work for a maximum of 1 calendar year in their assignment. Travelers are then required to take a minimum of a 6-month break before returning for another traveler assignment at the same facility.
Facility only accepts Hep B vaccines and will not take titers or declination
REFERENCES - Minimum of 2 supervisory references covering 1 year (6 months for CNA s) of employment total within the last three years within a comparable specialty/setting that they are booked. This can include Charge RNs / Leads.
If required for the assignment, Client agrees to supply Providers with communication devices (i.e. cell phone, pager, etc.) needed to perform the duties as assigned at no cost to Provider or Agency. If Client does not provide a mobile phone required for the assignment, Client agrees to pay Agency monthly, for all work-related cell phone use by each Provider, up to a maximum amount of $50.00 as invoiced by Agency. Providers will be responsible for submitting monthly cell phone bill directly to Agency.
Modules: • Modules are non-billable, average 16 hours spent. Over 20 hours requires PM approval • Modules are completed pre-start, and annually • Time spent on modules is self-reported, completion is recorded in Workramp transcripts
