A Home Care Visiting RN provides safe, holistic care to a population with multiple co-morbidities and advanced chronic illnesses. The RN educates regarding self-management and provides support to meet the needs of the patient, family, and community.
Submission requirements: • 1+ years of recent Home Health experience - REQUIRED • Case management experience - REQUIRED • Homecare Home Base EMR - REQUIRED • Previous travel experience - Highly Preferred • IV and venipuncture experience - Highly Preferred
