Special requirements: This is an Outpatient PT position and would prefer an applicants with some outpatient experience. Our clinic is a rural health clinic, treating most diagnosis; however, our biggest population is ortho (post op and general pain), neuro, falls/balance, and vertibular. We can be flexible with a 4x10 or 8x5 schedule, depending on applicant need.

Additional Unit Information • Interdisciplinary Support: - • Patient Diagnoses: We provide Outpatient PT services to patients primarily ages adolescent and up for a variety of conditions, including but not limited to orthopedics, post op ortho, ortho spine, neurological conditions (CVA, MS, Parkinson's), vestibular, falls/balance, persistent pain, and pelvic health.
Special Procedures/Unit Details: • WE have 45 min evaluation and treatment slots and build chart time into the schedules. We provide 1:1 care and do not double book.

PLEASE NOTE - Must have a vehicle for this assignment!
If required for the assignment, Client agrees to supply Providers with communication devices (i.e. cell phone, pager, etc.) needed to perform the duties as assigned at no cost to Provider or Agency. If Client does not provide a mobile phone required for the assignment, Client agrees to pay Agency monthly, for all work-related cell phone use by each Provider, up to a maximum amount of $50.00 as invoiced by Agency. Providers will be responsible for submitting monthly cell phone bill directly to Agency.
Modules: • Modules are non-billable, average 16 hours spent. Over 20 hours requires PM approval. • Modules are completed pre-start, and annually. • Time spent on modules is self-reported, completion is recorded in Workramp transcripts.
