Position Summary:
We are seeking a licensed and motivated Occupational Therapist (OT) to join our healthcare team. The OT will be responsible for assessing patients’ functional abilities, creating individualized therapy plans, and providing interventions to help patients achieve independence in daily living activities and improve overall quality of life.

Key Responsibilities:
• Perform patient evaluations to assess functional performance, self-care skills, and rehabilitation needs.
• Develop and implement personalized treatment plans based on patient goals and clinical needs.
• Provide therapeutic activities, exercises, and adaptive strategies to maximize independence.
• Monitor and document patient progress while ensuring compliance with state and federal guidelines.
• Educate patients and families on therapy goals, adaptive equipment, and home exercise/activities programs.
• Collaborate with interdisciplinary healthcare team members (PTs, nurses, physicians, speech therapists, etc.) to ensure coordinated patient care.

Qualifications:
• Active and current Occupational Therapist (OT) license in the state of practice (required).
• Graduate of an accredited Occupational Therapy program (OTD or equivalent).
• Strong clinical knowledge and patient-centered approach.
• Excellent communication, organization, and teamwork skills.
• Previous experience in SNF required.
