Unit Notes: Type of Facility: ALL Skilled Patients, Short term patients, no long term # of Beds/Residents: 24 beds, Private rooms EMR: Point Click Care Ratios: • RN/LPNs: 2 nurses (1:12) • CNA's: 2 CNAs (1:12) Required Certifications: CPR Must have their CPR prior to first day Common diagnosis / Types of patients: Sepsis, Femur Fx, NSTEMI, CKF, CHF, UTI Majority patients here to receive IV Antibiotics or therapy to gain strength to return home
Job Duties: • RN/LPN - Pass meds, wound care, foley/cath care, PEG tube feedings, ostomy care, trache care • RN will take care of IVs, PICC lines, mediports, etc
Experience or 'Must have' skills: • Are there IV's or titratable drips: IVs, PICC lines, Mediports, Central lines, IV Antibiotics/Blood/ IVFA • Are there Tele monitors: NO • Are RN's required to attend team conferences for acute inpatient rehab: N/A
DON/Charge Nurse/House Supervisor: Darla Kuri is DON; Always a charge nurse in facility on 2W. Manager is Wende Lightner; Clinical Coordinator is Ashley Conway Physician availability after hours: Physicians available to be called 24/7 Interdisciplinary Team: PT, OT, ST, Social Services, Case Managers, Business Office, Dieticians, Pharmacist, Medical Records all on site Is RT on-site 24/7: NO Oxygen Therapy /Equipment (ie Cpap, Bipap, High Flow): NO Pharmacy on-site: Pharmacist on site; medications received from hospital down the hill
Shifts & Scheduling: • No self scheduling; do not guarantee blocked schedule • 12 hour shifts (6:45-7:15), every other weekend required; scheduled according to facility needs; ALWAYS an RN in the facility Weekend Requirements: Every other weekend required Holiday Requirements: N/A Call or Standby Requirement: NO Floating Requirements: Travel float as needed, travelers will float before CORE staff Scrub Color/Dress Code: • RN's: navy blue nurses • LPN's: navy blue nurses • CNA's: gray scrub pants; any print top Unit/Department Specific Orientation: 2 shifts orientation Do you have a Separate COVID Unit: No private rooms -- enhanced droplet precaution for COVID Do you use PAPR or N95: N95 Require COVID Vaccine: No
