Mobile Health Team Inc., a staffing agency has openings for a Physical Therapist(PT) - Home Care. We are looking for someone with a passion for people.

Assignment Start Date:  11/24/2025

20 weeks | Days | 8-hour shift | 40 hours per week with 36 hours guaranteed

Physical Therapists needed to provide one-on-one therapy care for patients in their home setting! Looking for a candidate with no requested time off.

RESPONSIBILITIES

• Assess a person’s condition and develop individualized plans of care to help people reach their goals and maximize mobility and independence.
• Collaborate with interdisciplinary teams and colleagues to ensure a person’s needs are met.
• Demonstrate ability to perform standard therapy measurement techniques, including ROM, strength, and balance tests.
• Utilize a variety of therapeutic modalities to maximize the care provided to clients

EXPERIENCE/EDUCATION/QUALIFICATION

Experience:

• Home Health experience and OASIS experience are a must as this candidate will case-manage
• No RTO required

Licenses, Certifications, and/or Registration:

• Vermont PT licensure is required
• Basic Life Support certification is required

EMR System: McKesson EMR experience is strongly preferred

Shift:  M-F with weekend rotation
